Free/Reduced-Price Meal Applications and Direct Certification Notices for the 2014-15 school year are available at all schools. The middle and high school Free/Reduced-Price Meal Applications and Direct Certification Notices also will be available during registration on Aug. 19, 20 and 21.
If you received a Free/Reduced Meal Application, you are encouraged to complete the application and return it to the Food Service Office, 856 W. Cook Road, P.O. Box 1448, Mansfield, Ohio 44901 before school begins to ensure speedy processing.
If you want the result of the meal application shared with the school for the purpose of waiving school fees, etc, please complete and sign part 5 of the application in addition to part 6. Only one application per household is needed.
If you have received a Direct Certification Notice, please do not complete an application for that child. However, complete and return the purple-colored “Sharing Information with Other Programs” form which is enclosed with the notice. Please return the purple-colored form to the Food Service Office at the above address.
